As defined by the NASW, case management is:
βa process to plan, seek, advocate for, and monitor services from different social services or health care organizations and staff on behalf of a client. The process enables social workers in an organization, or in different organizations, to coordinate their efforts to serve a given client through professional teamwork, thus expanding the range of needed services offered. Case management limits problems arising from fragmentation of services, staff turnover, and inadequate coordination among providers. Case management can occur within a single, large organization or within a community program that coordinates services among settings (NASW, 2013).β
Iridescent Care Coordination offers two different types of case management.
Professional Guardianship & Payee Services Clients
Clients who receive professional guardianship services and payee services automatically receive case management. This is a short list of whatβs provided.
- This includes regular in-person bi-monthly meetings and home checks.
- Annual review of your care plan that was created after joining our family.
- Regular appearances at your Individual Plan of Service (IPOS) meetings, Individual Education Program (IEP) meetings, and other meetings as deemed important.
- Attendance at all necessary medical appointments (if requested by client) to ensure that your voice is heard.
- Appearances at all court cases (if needed and requested by client).
- Renewing medical benefits, including Medicaid, Medicare, and Social Security.
Independent Case Management Clients
We provide independent case management to clients who are interested in receiving services to assist with improving their physical and mental health, coordinating care, and community services through advocacy.
